NACOGDOCHES, TEXAS – Around 7:00 p.m. on August 28, 2015, 54-year-old California truck driver was driving northbound on US Highway 59 near Old Lufkin Road south of Nacogdoches, Texas. The semi-trailer that Rafael Polendo Hernandez was driving was hit from behind by a 2012 Chevrolet Traverse driven by Michael Zambo from Lufkin, Texas. Blaze Zambo, an infant passenger in the Chevrolet, died at Memorial Hospital from injuries suffered in the accident. Two six-year-old children who were also passengers in the Chevrolet were hospitalized following the accident. Six-year-old Richard Lira is still in critical condition at Children’s Hospital in Houston. Laura Saldaña, 23, from Lufkin was also a passenger in the Chevrolet. Ms. Saldaña died at the scene of the accident. Michael Zambo was also hospitalized following the crash.
